Site Risk Assessment Form

WORKSITE INDUCTION & RISK ASSESSMENT 

This form is to be discussed and completed by all staff to work on site throughout the project.  All workers joining the crew after the start of work must also be taken through the worksite induction risk assessment and control process.  All workers once inducted and aware of the worksite provisions, hazards and control measures must sign this form in the space provided.  The completed form must be held on site until the job is completed.  A new form must be completed for each worksite/job as detailed in the procedure.

THIS FORM IS TO BE FILLED OUT ON THE FIRST DAY OF WORK AND AMENDED AS NECESSARY THROUGHOUT PROJECT. IF INSPECTION REVEALS THAT WORK CANNOT BE PERFORMED SAFELY, DO NOT PROCEED UNTIL ACTION IS TAKEN TO ENSURE THE SAFETY OF ALL INVOLVED
